Schedule Details/Additional Information:
Inpatient pharmacy practice experience preferred. BCPS or other board certification preferred.This is a per diem position where shifts may vary between 1st and 2nd shift depending on department needs. There are no required weekends.

Major Responsibilities:

  • Demonstrates competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems. Independently evaluates patients for drug-drug, drug-food, drug-lab, and drug-disease interactions. Designs individualized drug therapy regimens including medication doses based on patient’s size, organ functions, and disease states.
  • Facilitates medication-related continuity of care at admission, transfer and discharge. Guides physicians, medication history team members, and nurses. Ensures that medication reconciliation processes are consistently followed and provides support for more complex cases.
  • Reviews and accurately interprets medication orders. Provides guidance to other pharmacists and physicians for order selection, entry and verification.
  • Demonstrates competency in dispensing and compounding medications consistent with good pharmaceutical practice including compounding technique, packaging, and labeling. Practices in compliance with department policies & procedures, as well as local, state and federal regulations. Acts as a resource to others. Provides input for department policy revisions and dispensing practices.
  • Ensures formulary adherence, recommends cost effective therapies to providers, and minimizes waste in daily practice. Identifies cost reduction opportunities.
  • Actively supports procedures and initiatives to prevent medication events. Reports events through appropriate channels. Actively participates in the development and implementation of department medication safety and quality improvement activities.
  • Orients/trains new pharmacy team members. Routinely works with interns, residents and students to guide daily activities. Serves as preceptor for interns, students, and/or residents. Provides direction to pharmacy technicians to ensure smooth workflow and service standards.
  • Demonstrates a commitment to continued individual professional growth.

Licensure, Registration, and/or Certification Required:

  • Pharmacist license issued by the state in which the team member practices.
  • Patient facing pharmacists are required to have two certifications:  Immunization and non-vaccine injectable to be granted from the state of WI or if from out of state, the program is approved by the accreditation council for pharmacy education (ACPE) or by the WI pharmacy examining board.  

Education Required:

  • Bachelor's Degree in Pharmacy.

Experience Required:

  • Typically requires 3 years of experience in pharmacy, including residency (years).

Knowledge, Skills & Abilities Required:

  • Specialty certification required (for external hires, certification must be obtained within one year of hire).
  • Maintains proficient skills in area of practice, demonstrating a commitment to continuing professional growth.
  • Strong computer skills.
  • Excellent communication and organization skills.
  • Excellent presentation, facilitation and training development skills.
  • Demonstrated decision making, team building, customer service, and conflict resolution skills.
  • Certification Addendum
  • Team members will maintain annual educational requirements for hospital based certifications (e.g.DNV, CARF or others).
  • Life support training courses may be required dependent on department discretion 

Physical Requirements and Working Conditions:

  • Must be able to stand for long periods of time.
  • Will occasionally lift up to 35 lbs. without assistance.
  • Must have functional speech, vision, and hearing.
  • Will be exposed to chemical hazards.
  • Operates all equipment necessary to perform the job.
  • Exposed to normal office environment.
